        Start with Dennis recommendations. 

So many different causes for something like this.  You will need to follow a list of checks. 

And if fuel filters and such were not changed in your memory then only good can come from changing them.

For me the ultimate cause of a similar issue was a bad brand new AFM from GW.  Many attempts to fix did not improve things until I used a AFM that a list member sent me through Dennis Haynes.  That gift saved the day! 

Van is running great for the last two years. 
Before that we tried ,

Wiring harness, New Temp 2 sensor, new O2 sensor and wire check. 
New throttle body. 
And bunches of grounds. 

It could be any of those things.  But for us, a new bad AFM was the sneaky culprit.
